Therapy Assistant (COTA) Contract, Skilled Nursing Facility
Position overview Seeking a Certified Occupational Therapy
Assistant (COTA) for a 13 week contract in a skilled nursing/rehab
setting. The COTA will provide rehabilitative services under the
supervision of an Occupational Therapist to help adult and
geriatric residents maximize safety and independence in daily
activities. Job type: Contract, 13 weeks (3/31/26 6/27/26)
Schedule: 8:00 AM 4:30 PM, 30 minute unpaid meal, minimum 35
hours/week guaranteed Setting: Skilled Nursing Facility (SNF) /
long term care and post acute rehab Rate: $42- $50 per hour Status:
New opening; non permanent traveler assignment Key responsibilities
Implement OT treatment plans developed by the supervising
Occupational Therapist, using therapeutic activities and exercises
to improve functional mobility, strength, coordination, and
cognition. Instruct and assist residents with ADLs and IADLs
(bathing, dressing, grooming, toileting, feeding, functional
mobility, home management tasks) including use of adaptive
equipment and compensatory techniques. Conduct observations of
residents ability to perform daily tasks and report progress,
barriers, and safety concerns to the supervising OT and care team.
Document treatment sessions, resident response, and functional
progress in the EMR accurately and in a timely manner, consistent
with facility, payer, and regulatory requirements. Provide
education and training to residents, families, and caregivers on
safe transfers, energy conservation, joint protection, and home
exercise programs to support carryover after discharge. Participate
in interdisciplinary team meetings and care conferences,
contributing to treatment planning and discharge recommendations.
Assist with environmental assessment and recommendations for
adaptive equipment, safety modifications, and positioning to
prevent complications and enhance function. Follow established
infection control, fall prevention, and safety protocols and
maintain a clean, safe treatment environment. Comply with state
practice act, professional standards, and supervision requirements
for COTAs. Qualifications Graduate of an accredited Occupational
Therapy Assistant program with current national certification
(COTA). Active, unencumbered state license as a COTA (or
eligibility to obtain prior to start). Current CPR/BLS
certification from an accepted provider. Preferred: At least 1 year
of recent experience in a skilled nursing facility, long term care,
or similar adult rehab setting; strong new grads with relevant SNF
clinical rotations will be considered. Proficiency with electronic
documentation systems and ability to meet productivity and
documentation expectations. Strong communication, teamwork, and
customer service skills with the ability to motivate and engage an
older adult population. Ability to meet the physical demands of the
role, including frequent standing, walking, bending, and assisting
with transfers and mobility.
